Event Research ReleaseI have read the Description of the Evaluation of Rendezvous Participation (opens in a new window) or it was read to me. I know the possible risks and benefits. I know that being in included in the evaluation is voluntary. I choose to be in the evaluation of this event. I know that I can withdraw at any time. I am at least 18 years of age and voluntarily grant permission for the described release and use of notes and comments regarding me at Rendezvous events. Please select one of the following choices to indicate your level of desired participation in the evaluation:

I consent to participate in ALL aspects of this evaluation. My pre-Rendezvous information, all event information, and follow-on event information can be used in this findings from the evaluation, and I agree (if requested) to participate in a follow-up interview in approximately 2-6 months which will be audio recorded. Video, still images, and audio collected at the event may also be used in the evaluation findings.
