Early Career Retrospective Study - Interview Consent Form

Description of project

The Early Career Workshop for Geoscience Faculty is currently conducting an NSF funded retrospective study to investigate its influence on past workshop participants.

As a part of this study, we would like to ask you some questions about your experience with the Early Career workshop. In order to document what is said during the interview, we be taking notes of discussions and will record the call for our records. The video files and notes will be kept in a restricted-access folder and will be used only for research purposes. The material will be used to inform a retrospective study of the Early Career project. When individual quotes are used to illustrate important points, your name will not be identified. In short, we guarantee that you will not be identified if your work is used for evaluation. There are no foreseeable risks resulting from your participation. Your participation in this research project is voluntary and you may leave at any time.

Terms of Consent

Your consent here indicates your approval to participate in the study. We seek your consent to use the interview for research purposes. The researchers will guarantee that the information obtained through all surveys will be kept confidential. This project has been granted IRB exempt status.

You understand that your participation is voluntary and you are free to withdraw consent and discontinue participation in this study, without having to specify a reason, at any time by notifying Ellen Iverson by e-mail eiverson@carleton.edu.
