In confirming my registration for this workshop, I agree that any audio recording or images (still or video) made of me during the workshop may be used on the project website, in printed project publications, and in materials developed by the project. I further agree that my contributions to creative works that emerge from workshop activities (e.g., discussions on the email list, contributions to the web page, summaries of group discussions, etc.) may be used by the workshop series, PIs, and conveners for development of future workshop materials, for scholarly reports, and for promotion of the workshop series. Attribution for original contributions will be made whenever possible.

Project EDDIE seeks to develop high quality teaching materials that are easy for instructors to adopt and adapt in their teaching. A series of webinars and workshops will help the project align resources with the community needs. We are conducting surveys and assessing workshop and webinar discussions to better understand 1) barriers and solutions for teaching quantitative reasoning with large data sets, and 2) the effect of the professional development on participants. These data will be incorporated into our research results in aggregate. Presentation of the material will be confined to project work groups, professional publications, and conferences. When individual quotes are used to illustrate important points, your name will not be identified. There are no foreseeable risks resulting from your participation.

Consent

I have read the Description of the Evaluation of Participation and Implications of Consent (opens in a new window) or it was read to me. I know the possible risks and benefits. I know that being in this study is voluntary. I choose to be in this study. I know that I can withdraw at any time. I am at least 18 years of age and voluntarily grant permission for the described release and use of my notes and comments.







By submitting my registration form, I agree that I will attend the entire workshop. I will also complete the assigned pre-workshop assignments, and be prepared for discussions at the workshop. I also accept responsibility to cover any costs incurred should I withdraw from participation in the workshop after October 14th, 2019. I understand that arrangements have been made on my behalf that have associated costs (e.g. meals, materials). Exceptions will only be made in cases of extreme personal hardship.
