This website provides access to the Behavioral Risk Factor Surveillance System (BRFSS) mapping tool. The BRFSS is a telephone survey conducted by the Center for Disease Control and Prevention which tracks health risks in the United States. Maps created on the BRFSS website illustrate health risks at national, state, and local levels. Categories that may be mapped include asthma, diabetes, immunization, demographics and more.
