OOI Ocean Data Development Workshop

July 22-26, Asilomar Conference Center, Pacific Grove, CA

Consent and First Roadcheck Survey

As a participant in this Data Development Workshop, you are being asked to take part in an educational research project conducted by the Ocean Observatories Initiative (OOI) evaluation team. The evaluation will examine the effectiveness of the workshop in fostering a community and supporting the uptake of OOI educational resources.

If you agree to take part in this research, we will analyze materials you submit prior to the workshop (e.g., application forms, vlogs) as well as materials you submit as part of the workshop (e.g., roadcheck surveys, end-of-workshop surveys). This information will be used to improve future workshop sessions, to assess the success of the workshop in meeting program goals, and to develop future evaluation tools. Follow-up interviews will be conducted with a small number of workshop participants. We will conduct these interviews via telephone approximately two to six months after the workshop. If you do not wish to be audio-taped, you should not participate in this part of the evaluation.

Any information you provide for this evaluation will be used only for research purposes and in ways that will not reveal who you are. There are no foreseeable risks resulting from your participation. Your participation in this workshop is voluntary and you may leave at any time.

If you have any questions about this research project, please contact Dr. Ellen Iverson at eiverson@carleton.edu.

The chair of the Institutional Review Board, Dr. Kathleen Galotti, can be reached by mail at Carleton College, Northfield, MN 55057, by phone at (507) 222-4376, or by email at kgalotti@carleton.edu. * required

In participating in this workshop, I agree that any audio recording or images (still or video) made of me during the workshop may be used on the project website, in printed project publications, and in materials developed by the project. I further agree that my contributions to creative works that emerge from workshop activities (e.g., contributions to the website, summaries of group discussions) may be used by project leaders for the development of future workshop materials, for scholarly reports, and for promotion of the workshop series. Attribution for original contributions will be made whenever possible.

Purpose: Please reflect on today's workshop sessions and let us know what worked and what needs improvement. Your input is valuable to the project leaders as they plan for tomorrow and for future workshops. We would like you to enter your name below. This will improve our ability to evaluate the workshop by allowing us to link your responses from this survey to other evaluation instruments. Responses will be presented to the project leaders only in aggregate; your name will not be associated with your responses.
