Optional Pre-Workshop Events on Monday Afternoon: Two concurrent workshop sessions are planned for Monday. Please indicate your choices for these sessions below. Teaching Statement Sessions
We will offer concurrent sessions on teaching statements as a way to articulate your teaching goals and highlight your teaching accomplishments. There will be two choices for this session. For those who do not yet have a draft of a teaching statement, we will have a formal session on developing your teaching statement. For those who submit a draft of a teaching statement by May 20, we will have a discussion and peer review session (workshop leaders will also offer their feedback during this session). Recognizing that this is not a firm commitment, please indicate in which session you are more likely to participate:
Research Statement Sessions

Teaching and Professional Interests: Please indicate your level of interest at this point in time in working in each of the following settings (after finishing your graduate and/or postdoctoral work). This information is for our use and results will be reported only in aggregate.
Optional Post-Workshop Events on Thursday morning: Three concurrent workshop sessions are planned for Thursday morning. Please indicate your choices for these sessions below. If the number of people selecting a specific session isn't sufficient, we will cancel that session and give you the opportunity to revise your selection. Please consider your travel plans before you sign up for these sessions, taking into account the time you will need to get to the airport if you are flying out.
In confirming my registration for this workshop, I agree that any audio recording or images (still or video) made of me during the workshop may be used on the project website, in printed project publications, and in materials developed by the project. I further agree that my contributions to creative works that emerge from workshop activities (e.g., discussions on the listserv, contributions to the web page, summaries of group discussion, etc.) may be used by the workshop series, PIs, and conveners for development of future workshop materials, for scholarly reports, and for promotion of the workshop series. Attribution for original contributions will be made whenever possible.
I also accept responsibility to cover any costs incurred should I withdraw from participation in the workshop after June 1, 2011. I understand that arrangements have been made on my behalf that have associated real costs (e.g. logistics, materials), and in many cases, my early confirmation has secured a place that has been denied to other interested colleagues. Exceptions will only be made in cases of extreme personal hardship.
